How does a farm work?

When I use the overlay to find a good place for a farm I tend to place the farm on the best spot - the field with the highest output e.g. 60%. So far so good, but when I put the farm right on the next field, which have an output of just 50% the game tells me I will have a better output per hour than with the 60% field.

So here comes the question... Does the farmhouse itself not count to the ouput value and block the good field with, lets say a bunch of nanites? So would it it be better to place the farm house on the bad field to have the farmland itself on the good fields?

That's correct; the farmhouse itself does not count toward total productivity, only the farmed tiles around it.  We recommend placing it wherever it states that the productivity is highest, and the farm itself will automatically pick the optimal tiles around itself to farm.
